Dr. Bonafede graduated Magna Cum Laude in 1987 from John Carroll University with a Bachelor of Science.  In 1991 he graduated Magna Cum Laude from medical school at the State University of New York at Buffalo School of Medicine and Biomedical Sciences.  His post-doctoral work includes a General Surgery internship, Otolaryngology residency, and a fellowship in Head and Neck Otolaryngology and Microvascular Surgery, all at the Cleveland Clinic Foundation in Ohio.  He currently practices in Bay Shore and East Patchogue, New York.

Dr. Cussatti received his Bachelor of Arts in Natural Sciences from Fordham University in New York, followed by his medical degree at Howard University College of Medicine in 1997.  He completed his General Surgery residency at Columbia University, College of Physicians and Surgeons in 2002, and went on to train at the University of Pennsylvania.  He is currently Director of Minimally Invasive Surgery & Bariatrics at Good Samaritan Hospital in West Islip, New York.

Luis Fandos, MD

Dr. Fandos was born in Argentina where he finished his studies as Medical Doctor at the University of Rosario.  He went on to complete his anesthesia and critical care residency at Mount Sinai Medical Center in New York City, and fellowship training in Interventional Pain Management at St. Luke’s-Roosevelt of Columbia University.  He went on for additional subspecialty training at Beth Israel Medical Center in Manhattan.  Dr. Fandos later joined Dr. Craig Shalmi and began New York Pain Consultants on Long Island.  He is currently on staff in the Division of Pain Management at Good Samaritan Hospital Medical Center where he participates in the teaching of medical residents wishing to learn about this new field.  He is serving as the Director of the Division of Pain Management at Southside Hospital in Bay Shore, a member of the North Shore-LIJ system since November of 2006. Dr. Fandos is currently involved in several research projects that focus on patients with chronic pain due to different conditions focusing on new techniques that can potentially provide these patients with prolonged sustained relief of pain and improved quality of life.

Dr. Litman earned his Bachelor of Science in Biology from Rensselaer Polytechnic Institute.  He attended medical school at New York Medical College in 1987.  He completed his internship in internal medicine at Stamford Hospital in Connecticut and went on to complete his Clinical Anesthesia residency at Westchester Medical Center in New York.  In 1993 he completed an Anesthesiology Preceptorship at Emory University in Georgia.  He is currently Director of All Island Pain Consultants in Bay Shore.  Dr. Litman is a Diplomate and Examiner for the American Board of Interventional Pain Physicians, as well as a Fellow of International Pain Practice at the World Institute of Pain.

Dr. Marzec is an Orthopedic Surgeon working in the Bay Shore area.  He graduated from HOFSTRA University with honors in 1978, and from Hahnemann Medical College in Philadelphia in 1982.  He completed his internship and residency at Maimonides Medical Center in New York between 1983 and 1987, and completed a Sports Medicine Fellowship at the Hughston Sports Medicine Hospital in Columbus, Georgia in 1988.  Dr. Marzec is the Medical Director for Riptide Lacrosse, the Team Physician for the Harlem Globetrotters Basketball team as well as for the Long Island Ducks Baseball team.  He serves as Team Physician for the West Islip High School, Babylon High School and the East Islip High School, and is also the Chief Medical Officer of the West Islip School District.  He holds the title as the Director of Research and Development for Prostar Athletics, and is a Fellow of the American Board of Orthopaedic Surgery, the American College of Surgeons, the American Academy of Orthopaedic Surgeons, the American Orthopaedic Society for Sports Medicine, the Arthoscopy Association of North America and the Hughston Society.

Sanford Ratzan, MD

Dr. Ratzan earned his Bachelor of Arts from Hamilton College and completed medical school at Columbia University’s College of Physicians and Surgeons in 1965.  He completed his internship in General Surgery, a residency in General Surgery, and a residency in Orthopaedic Surgery at St. Luke’s Hospital in New York City between 1965 and 1972.  During this time he spent two years in the U.S. Army, and served in Vietnam 1967-1968.  Dr. Ratzan is Chief of Orthopedic Surgery at Good Samaritan Hospital Medical Center.  He is Attending Orthopedic Surgeon at Southside Hospital in Bayshore, and St Luke's Hospital, Columbia University division, NYC. Dr. Ratzan is Team Physician of Long Island Ducks Baseball Club and various Long Island high school teams. He has served as Orthopedic Consultant to the New York Yankees. He is a Police Surgeon for the NYPD.  Dr. Ratzan is a Fellow of the American Academy of Orthopedic Surgeons, the American College of Surgeons, the Arthroscopy Association of North America, and the American Orthopedic Society of Sports Medicine.

Dr. Ruotolo was a top graduate at the New York Medical College School of Medicine in 1995. He completed an orthopaedic residency program at the SUNY at Stony Brook in 2000. After his residency he underwent fellowship training in sports medicine and shoulder surgery at the prestigious Sports Clinic of Laguna Hills, California under the tutelage of Wesley Nottage, MD. Dr. Ruotolo is board certified in orthopaedic surgery and is a fellow of the American Academy of Orthopaedic Surgeons. As an Associate Master Instructor of Arthroscopy for the Arthroscopy Association of North America, Dr.Ruotolo actively teaches other orthopaedic surgeons advanced arthroscopic skills in shoulder surgery.  He currently serves as the Chairman of the Department of Orthopaedics at Nassau University Medical Center.

Dr. Shalmi earned his Bachelor of Science in Chemistry from Union College.  He attended medical school at Albert Einstein in New York City and graduated with distinction for research in oncology.  He completed his internship in surgery through Tufts University School of Medicine and went on to Harvard University School of Medicine to complete his residency in anesthesia and critical care.  He remained at Harvard for fellowship training in Pain Management and completed this in 1997.  He relocated to Long Island in 1999 where he began “New York Pain Consultants”, a state-of-the-art multidisciplinary group serving Long Islanders in the full spectrum of pain problems.  He is currently the Chairman of the Division of Pain Management at Good Samaritan Hospital Medical Center in West Islip.  He is board certified in Anesthesia and Pain Management. He is published and is a member of numerous professional organizations.

Dr. Simonsen is an Orthopedic Surgeon native to Long Island and works in the Bay Shore area.  Prior to graduation from the George Washington University School of Medicine and Health Sciences in Washington, DC in 1976, he received his undergraduate degree from St. Lawrence University in 1972.  He completed his internship and residency in General Surgery at George Washington University Hospital between 1976 and 1978, and his fellowship in Orthopedic Surgery in 1981 at the same hospital.  In the past, he has served as Team Physician to St. John the Baptist High School.  Currently he is the Assistant Chief of Orthopaedic Surgery at Good Samaritan Hospital Medical Center, and he is on the Consulting Staff at St. Catherine of Siena Medical Center in Smithtown, NY.  Dr. Simonsen is also a Fellow of the American Academy of Orthopaedic Surgeons.
