As a patient, it is your right to have certain information provided to you prior to your surgical procedure in writing and verbally. These patient’s rights are extended to the patient’s representative or surrogate, as appropriate.

South Shore Surgery Center has a patient transfer agreement in place with Northwell Health Southside Hospital located at 301 East Main Street, Bay Shore NY 11706.
